Primary Caregivers are the glue that holds everything together in a family. We are expected to remember birthdays, plan events, and get everyone in the family prepared for daily routines as well as major life events - all while scheduling appointments, attending meetings, working and making time for friends and self-care. We do this for our children, our parents, our spouses and when we have time, for ourselves. CareForward Planners can help carry this load with you. We have two digital tools that will cut down on the time spent tracking down information and explaining how you do what you do!
One Portal
The Caregiver Portal contains important documentation from each of your loved one’s providers. When you need to challenge a denial, prove eligibility for services, track down receipts for reimbursements - the Portal holds it all in a secure online platform that only you have access to. You can grab any document you need from your phone. You can upload documents that you get in the mail or while attending a meeting simply by taking a picture of it and sending it to the portal and it automatically saves in the correct folder. CFP sets up folders for you that are organized based on disability service categories. Your IEPs, IPPs, NOAs and correspondence will have a dedicated home. If it is not there - they did not send it!
One Plan
One Plan
The Caregiver Guide pulls out all of the information you need from those documents, summarizes it and makes it accessible from your phone or for anyone who is part of the support team. Anytime you are bringing in help, they can easily step into your shoes because the instruction manual is there.
Transition Support
The CareForward Planners team will support you and your family through one of the most important decisions you will make, how will your loved one be cared for as they get older. Our experience navigating disability service agencies will support the transition of the next family member or care team into the role of managing your loved ones affairs.
Our Process
Meet with the family for a review of the Caregiver Guide, input and revisions before final delivery.
Record Review
CFP will review the most recent individualized education program (IEP), individual program plan (IPP), individualized plan for employment (IPE) assessments, behavior plans, and any other document that will help us learn your child’s needs, routines and life goals.
Planning Interview
Meet with the disabled adult, the caregiver and the next caregiver (provided that person has been identified). During this meeting we will use information gained from the document review to discuss building a team of support, selecting a successor caregiver, establishing funding sources for the caregiver to support the loved one, and independent lifestyle plans.
